PCOS Isn't Just About Your Ovaries. Here's Why Researchers Are Rethinking It.

For decades, millions of women have been told they have Polycystic Ovary Syndrome (PCOS) even though many never had cysts on their ovaries. The name itself has created confusion. It places the focus on the ovaries, when in reality, the condition reaches far beyond them.

In fact, a recent publication by an international group of experts has proposed renaming PCOS to Polyendocrine Metabolic Ovarian Syndrome (PMOS) to better reflect what clinicians and researchers have come to understand over the past several decades: this is not simply an ovarian condition. It is a complex disorder involving multiple endocrine pathways, metabolism, and ovarian function working together—not in isolation.

While this proposed name has not yet replaced PCOS in clinical practice, I believe it represents an important shift in how we understand and ultimately care for women living with this condition.

Because when we change the way we understand a diagnosis, we change the questions we ask.

Instead of asking,

"How do we regulate your period?"

We begin asking,

“Why has your body become dysregulated in the first place?"

And that's a much more powerful question.

PCOS Is Much Bigger Than Irregular Periods

Yes, PCOS can present with irregular or absent cycles, acne, unwanted hair growth, infertility, or ovarian changes seen on ultrasound. But that's only part of the story.

Underneath these symptoms are often deeper physiological processes involving:

  • Insulin resistance

  • Blood sugar dysregulation

  • Chronic low-grade inflammation

  • Altered stress hormone signaling

  • Brain-ovary communication

  • Metabolic dysfunction

  • Cardiovascular risk

  • Changes in androgen production

In fact, many women with PCOS have insulin resistance even if they are not overweight.

This is one reason I often say:

Hormones don't exist in isolation.

Everything is connected.

The Missing Piece

One of the greatest frustrations I hear from women is this:

"I was put on birth control... but nobody ever explained why this was happening."

Birth control can absolutely have a role for some women.

So can medications like metformin.

But neither automatically answers the question of why your hormones became dysregulated.

That "why" matters.

Because hormones respond to the environment we create inside our bodies.

They respond to:

  • Blood sugar

  • Sleep

  • Stress

  • Inflammation

  • Nutrition

  • Movement

  • Gut health

  • Nervous system regulation

  • Environmental exposures

  • Genetics

When we ignore those factors, we often end up chasing symptoms instead of restoring balance.

Ancient Medicine Has Always Looked at the Whole Picture

One of the reasons I fell in love with Traditional Chinese Medicine and Ayurveda is because neither system has ever viewed reproductive health as separate from the rest of the body.

In Traditional Chinese Medicine, menstrual health reflects the health of the entire organism.

Digestion influences Blood production.

Stress affects the smooth flow of Qi.

Sleep replenishes the Kidneys.

Inflammation, circulation, and emotional wellbeing all influence the menstrual cycle.

Ayurveda offers a similarly interconnected view.

It examines:

Digestion (Agni)

Tissue nourishment (Dhatus)

Constitution (Prakriti)

Imbalances (Vikriti)

...and how daily routines, seasonal rhythms, and stress shape overall health.

Different language.

Different framework.

But remarkably similar principles.

Modern research is increasingly showing that metabolism, inflammation, the nervous system, and reproductive hormones are deeply intertwined—a perspective these traditional systems have embraced for centuries.

So Where Do We Begin?

If you've been told you have PCOS or you've struggled with symptoms that sound familiar you don't have to overhaul your life overnight.

Small, consistent habits can have a meaningful impact on metabolic and hormonal health.

Here are a few places to start:

✔ Prioritize protein with breakfast to support blood sugar regulation.

✔ Take a 10–15 minute walk after meals to improve insulin sensitivity.

✔ Build muscle through resistance training to support metabolic health.

✔ Prioritize sleep as one of your most powerful hormone therapies.

✔ Eat enough fiber from vegetables, legumes, fruit, nuts, and seeds to support gut health and healthy estrogen metabolism.

✔ Find practices that help regulate your nervous system - whether that's meditation, breathwork, yoga, time in nature, acupuncture, or simply slowing down.

Remember, these habits aren't about perfection. They're about creating an internal environment where your hormones have the support they need to function well.

This Is Just the Beginning

Whether the name remains PCOS or eventually evolves to PMOS, one thing is becoming increasingly clear:

Women's hormones cannot be understood by looking at the ovaries alone.

They are influenced by

Metabolism.

Stress.

The gut.

Inflammation.

Sleep.

Nutrition.

Movement.

And the rhythms that govern the entire body.
